At SleepMS we are contracted with All major Health Maintenance Organizations (HMO) and All major Preferred Provider Organizations (PPO) for physician and clinical services. We also participate in government programs such as Medicare and MediCal.

The benefits for each insurance listed may be different for Clinic Services such as CPAP equipment dispensing and Consultation - Follow-up visits to see Dr. Clerk. You are advised to always check under your individual health plan, insurance or network to confirm whether Dr. Clerk is an accepted provider under your individual health plan network.

 

An In-Network Provider Can Save You Money

 

Choosing an in-network provider can save you money. If you choose to have your sleep study or CPAP device set-up with an out-of-network provider, it could cost you well over $1000 for each service. Alternatively, choosing an in-network provider can result in you paying a fraction of that cost. You may even pay nothing at all!

Insurance Coverage: What to Keep in Mind 

 

When it comes to your health insurance coverage, there are a few pieces of information that are important to remember, including:

• In order to take part in a sleep study, you must be referred by a primary care physician or specialist.

• Once you’ve been referred, your health insurance will typically cover your sleep study if it is deemed medically necessary.

• HMO insurance plans require authorization. Once your insurance has been properly authorized, we can schedule your service.

• Many PPO insurances now also require authorization. We will request authorization from your insurance provider before providing your service.
